This section provides a summary of the largest disparities in hospital claims-based health outcomes and health factors between different demographic groups within defined geographic areas. Users can specify individual or groups of counties most reflective of their primary services area, in addition to selecting two demographic groups for comparative purposes.

Community Health Needs Assessments

A community health needs assessment describes the community a hospital serves and identifies the health needs of the population. Nonprofit hospitals are mandated to conduct a CHNA every three years and adopt an implementation strategy to meet the community health needs identified in the assessment.

Along with the Health Equity Dashboard above, stakeholders also can utilize the exploreMOhealth website to learn more about the health in their communities. exploreMOhealth is one of the most unique health-related datasets in the country, offering both county- and ZIP code-level data. By examining targeted health data specific to a particular county or ZIP code, we are able to identify issues and take action to help create and sustain a healthier Missouri.

Infographics

Health disparities and social inequities are ubiquitous in Missouri and throughout the U.S. They occur across diverse populations and geographies, and their universal common denominator is SDOH. A set of infographics shows the influence of health equity among Missourians.
