Vicki works and thinks outside the box. She is a care manager in our small community, covering several counties in her role. She has been a nurse for 35-plus years in this community. Vicki currently works outside the hospital on improving patient outcomes and reducing readmissions. She manages patients who have been identified by their primary care providers as needing extra support. She individualizes a plan of care for each of them, meeting them “where they are.”

Vicki sets up medications, picks up supplies and ensures that these complex patients receive follow-up care. She also manages tasks that are not required, dedicating herself to patient success. She builds trust with those she serves through caring interactions. Despite her allergies, she even has helped with patients’ pets. Nothing deters her from helping others. Vicki is a huge proponent of rural health care and a cheerleader for the community. She advocates, volunteers and heals people with her time and huge heart.
