Post-Acute Care Constituency Group
Post-acute care refers to a range of medical care services that support the patient’s continued recovery from acute illness and chronic disease management.
Home health, hospice, outpatient services, swing-bed services, distinct-part skilled and long-term nursing facility care are some of the post-acute continuum of care services offered by Missouri hospitals. In addition, hospitals work with community providers of those services to ensure safe transitions of care to meet the health needs of the patient.
MHA meets the unique needs of these providers through representation and advocacy; participation in public policy development; and communication, education and management strategies. MHA also collaborates with national organizations to lay the foundation for aligned positions on behalf of these care providers.
