The Centers for Medicare & Medicaid Services mandates the use of Care Plans as a tool to develop appropriate interventions based on the individual patient’s identified needs and to provide a pathway toward meeting health care goals. Often, regulatory surveys are identifying shortfalls with health care organizations' Care Planning processes and documentation practices. During this webinar, we will identify these common pitfalls and provide recommendations on mitigation efforts to ensure your patients' Care Plans meet regulatory requirements.
