Part III: Community Partnerships, Population Health & Community Health Needs Assessments

As the landscape in health care continues to change, integrating population health in care delivery has proven to be one of the most important strategies to improve the health outcomes of individuals, communities and populations. Understanding the importance of addressing clinical and nonclinical factors of health will be key to building a strong population health program. From a regulatory standpoint, not-for-profit hospitals are required to complete a community health needs assessment triennially to help identify gaps in health outcomes within their communities, thus helping them target interventions where they are needed most.

Attendees will learn the importance of using innovative platforms, such as the exploreMOhealth website and health equity dashboards, to understand the unique needs of their communities and to help them formulate viable strategies. This session also will highlight why it is important for hospitals to work with local partners to leverage one another’s strengths.
