Disruption is the new norm, and hospital case management is no exception. As hospital leaders make the 180 degree turn from volume to value, HCM leaders and their executive sponsors must transform their legacy HCM models to one that meets the expectations of the new value-based marketplace. This one-day program will provide a fundamental understanding of the changes taking place in hospitals across the country and the ongoing shift to care coordination.
Upon completion of this program, participants will be able to:
- explain the impact that the transition from volume to value has had on the hospital industry
- identify alternative models of hospital case management programs, and discuss the advantages/challenges among alternative practice models
- discuss the transformation from case manager to care coordinator
- examine the difference between utilization review and resource management
- describe the difference between process outcomes and patient-reported outcome measures
