Purpose

Disruption is the new norm, and hospital case management is no exception. As hospital leaders make the 180 degree turn from volume to value, HCM leaders and their executive sponsors must transform their legacy HCM models to one that meets the expectations of the new value-based marketplace. This one-day program will provide a fundamental understanding of the changes taking place in hospitals across the country and the ongoing shift to care coordination.


Objectives

Upon completion of this program, participants will be able to:

  • explain the impact that the transition from volume to value has had on the hospital industry
  • identify alternative models of hospital case management programs, and discuss the advantages/challenges among alternative practice models
  • discuss the transformation from case manager to care coordinator
  • examine the difference between utilization review and resource management
  • describe the difference between process outcomes and patient-reported outcome measures

Stefani Daniels spent her career in the executive suite of hospitals and is the founder and managing partner of Phoenix Medical Management, Inc. She is the co-author of The Hospital Guide to Contemporary Utilization Review, 2nd edition and the popular text The Leader’s Guide to Hospital Case Management. Daniels is a contributing author to the second and third editions of the Case Management Society of America’s Core Curriculum for Case Managers. She serves on the editorial boards of Lippincott’s Professional Case Manager and HCPros Case Management Monthly, and is a popular speaker on contemporary hospital and health system case management.
