Purpose

There’s a new health care marketplace, and it’s changing almost daily, adding new payment systems, new regulations and new care delivery models. There has been no indication that the momentum toward reimbursement based on value is changing. Nor has the shift to population health slowed down. Case management transformation will better position organizations to achieve results that are essential for success in a value-based environment. 

This comprehensive workshop is designed to achieve two goals: to explore the activities generally assigned to traditional case management departments and to provide an overview of the changing market and the strategies hospital leaders use to transform traditional case management departments into care coordination programs.


Objectives

Upon completion of this program, participants will be able to:

  • describe the impact that the changing marketplace has on case management programs
  • discuss the importance of ensuring there is consensus of the vision and purpose of a care coordination program
  • discuss why proactive advocacy is the care coordinator’s primary ethical obligation and how it is translated into practice
  • explain the value of using predictive screening techniques to identify targeted populations
  • describe the population health model and the position of acute care coordination
  • review the key issues governing utilization review activities

Marianne McHale Ramey is a specialist in strategic plan development and program execution, and she is recognized as a highly effective transformation project manager among her many clients. She has served as interim director/project manager of care management and utilization review at several hospitals. McHale Ramey has authored several articles on hospital case management and is the co-author of the popular book, “The Leader’s Guide to Hospital Case Management.”


Continuing Education
