What is it?
Readmission to the hospital within 30 days for patients who were diagnosed with congestive heart failure. CHF is a chronic progressive condition that affects the pumping power of heart muscles. While often referred to simply as “heart failure,” CHF specifically refers to the stage in which fluid builds up around the heart and causes it to pump inefficiently.

How's it measured?
Measured as a percent of readmissions for adult inpatient discharges within 30 days for CHF.
