Acute Myocardial Infarction (AMI)

AMI Chart

AMI Dashboard What is it?
Readmission to the hospital within 30 days of an acute myocardial infarction. AMI is the medical name for a heart attack. A heart attack is a life-threatening condition that occurs when blood flow to the heart muscle is abruptly cut off, causing tissue damage. This usually is the result of a blockage in one or more of the coronary arteries.

Proactively identifying patients hospitalized with AMI at high risk for readmission could help prevent 30-day readmissions by focusing on and implementing targeted interventions.

How's it measured?
Measured as a percent of readmissions for adult inpatient discharges within 30 days for AMI.
