Audience

This course provides a broad — but more specific — overview of contemporary case management and is suited for leadership and front-line staff directly or indirectly responsible for any aspect of a patient’s progression of care through an acute episode and beyond. This includes the ED leadership team, social service staff, utilization review specialists, nurse managers, revenue cycle representatives, hospitalists, care/case managers, admissions and bed management personnel, quality/performance improvement professionals, physician advisors, contract coordinators, physician relationship managers, the C-suite team and board representatives. Program content is applicable to large academic teaching facilities, community hospitals, long-term acute care hospitals, rehab facilities, and critical access hospitals.


Purpose

Hospital case management is a compilation of several services the hospital typically provides under its case management program umbrella. But, as the environment quickly changes, many hospital case management programs are transforming into continuum-based care coordination programs to support new payment models and new outcome expectations. This advanced boot camp is a more detailed version of last year’s introductory boot camp and will address the specific components and how they inter-relate and constantly evolve based on marketplace demands.

Despite the drastic shift and uncertainties in the U.S. political landscape, health care experts expect the drive to reduce health care costs, increase efficiency and demonstrate value, to remain steady or intensify. After a brief overview of the current health care industry and discussion about the uncertainties of the future, the program will drill down on specific services, such as access management, care coordination, utilization review, alternative structures, operations to help selected patients navigate across the continuum, the use of predictive analytics and application of populations health concepts, the regulatory environment and compliance gaps, and the ongoing movement to measure performance to track return on investment.

At the conclusion of this program, participants will be able to assess the effectiveness of their facility’s case management program and care management processes, and will gain new insight on how to plan for the future.


Objectives

Upon completion of this program, participants will be able to:

  • describe the impact that the changing marketplace has on case management programs
  • discuss the importance of ensuring that there is a consensus of the vision and purpose of a case management program among hospital leadership
  • explain how the current cost of care has influenced utilization review and resource management activities
  • explore options on how to structure and operationalize a case management program across the continuum
  • differentiate utilization review from utilization management activities
  • explain the value of using predictive screening techniques to identify targeted populations
  • review some of the key regulations governing utilization review practices
  • discuss why proactive advocacy is the care manager’s primary ethical obligation and how it is translated into practice
  • explain the value of interdisciplinary coordination
  • describe metrics that can be used to evaluate the effectiveness of the care management program

Stefani Daniels spent her career in the executive suite of hospitals and is the Founder and Managing Partner of Phoenix Medical Management, Inc. She is the co-author of the recently published “The Hospital Guide to Contemporary Utilization Review” and the popular text, “The Leader’s Guide to Hospital Case Management.” Daniels is a contributing author to the second edition of the Case Management Society of America’s “Core Curriculum for Case Managers.” She serves on the editorial boards of Lippincott’s Professional Case Manager, HCPros Case Management Monthly and CMSA Today, and is a popular speaker on contemporary hospital and health system case management.
