In July 2002, The Joint Commission began requiring accredited hospitals to collect and submit core measure data.

Core Measure Sets

  • acute myocardial infarction (AMI)
  • congestive heart failure (CHF)
  • community-acquired pneumonia
  • pregnancy and related conditions
  • surgical care improvement project
  • pediatric asthma
  • hospital inpatient psychiatric services (HIPS) (pilot)

What does this mean?

If your hospital treats AMI, CHF, pneumonia and/or pregnancy patients

Beginning with 2008 discharges, acute care facilities must select a performance measurement system vendor to process and transmit four core measures to The Joint Commission.

Core Measures

  • acute myocardial infarction (AMI)
    • aspirin at arrival
    • aspirin prescribed at discharge
    • ACE inhibitor for left ventricular function
    • adult smoking cessation advice/counseling
    • beta blocker at arrival
    • beta blocker prescribed at discharge
    • time to thrombolysis
    • time to percutaneous transluminal coronary angioplasty
    • inpatient mortality
  • congestive heart failure (CHF)
    • discharge instructions
    • left ventricular function assessment
    • ACE inhibitor for left ventricular function
    • adult smoking cessation advice/counseling
  • community-acquired pneumonia
    • oxygenation assessment
    • pneumococcal screening and/or vaccination
    • blood cultures
    • adult smoking cessation advice/counseling
    • pediatric smoking cessation advice/counseling
    • antibiotic timing
  • pregnancy and related conditions
    • vaginal birth after cesarean section
    • neonatal mortality
    • third or fourth-degree laceration
  • surgical care improvement project
    • prophylactic antibiotic within one hour before incision
    • prophylactic antibiotic selection
    • prophylactic antibiotics discontinued within 24 hours after surgery
    • surgery patients on beta blocker before admission who received a beta blocker during the perioperative period
    • surgery patients with recommended venous thromboembolism prophylaxis ordered
    • surgery patients who received timely venous thromboembolism prophylaxis
    • betablocker before admission
  • pediatric asthma
    • relievers for inpatient asthma
    • systemic corticosteroids for inpatient
    • home management POC given to patient/caregiver
  • hospital inpatient psychiatric services (pilot)
    • assessment of violence risk, substance use disorder, trauma and patient strengths completed
    • hours of restraint use
    • hours of seclusion
    • patients discharged on multiple antipsychotic medications
    • discharge assessment and after care recommendations sent to next level of care providers upon patient's discharge
